I outline first, then turn the outline into the program. Items on the outline are 'actions' or 'data', data goes to the top of the outline, actions follow.
I hope that's clear and that it will work for you, it works like a charm for me, even for very large projects.
Outline editors are a dime a dozen, not sure which ones you could use on the mac though...
