Well, in the industry usually fewer eyes are looking at the code than in open source. Nobody strives to make it perfect overall, people are focused on what is important. Numbers, performance, stability, priorities depend on the project. There are small tasks approved by managers, developers aren't interested in doing more. Bigger company works the same, it has just more projects.
