My spam filter for cold outreach is simple: if it opens with "Hi <real name>" or "Hi <HN user name>", there's a good likelihood it's a human.
If they don't know my name, they don't even know where they got my email from, so probably spam, however intelligible it looks.
It's the same in the age of spam calls. If it's a mobile phone and the person behind didn't even bother to introduce themselves via SMS/WhatsApp, I don't pick up.
