Anyone that's using PQC as more than a curiosity would presumably take a belt-and-suspenders approach by using multiple rounds of different PQC as well as classical cryptographic algorithms, hopefully carefully selected so that they rely on different mathematics and therefore do not share the same theoretical weaknesses.

It's unlikely. There's a diversity of mathematical approaches represented, including some who's underlying systems have been studied for the last 50 years
Depends if it was before or after they started using them to secure important data. Before - it's no big deal, just the safeguards working as they should. After - that would be bad.
