My best guess is that he was going to have plastic surgery in Jaipur. There are several plastic surgeons right near the Fortis hospital. Maybe to alter his appearance. And then the surgery went badly.

Or he had the fake death certificate and bought a fake body in Jaipur. Then did plastic surgery and scooted out of there.
