Generally rsync to a mirror-image hardware box in the same location. Everything is backed up so any fault/failure on the primary machine can be recovered (in theory) via the hot-standby machine.
Critical user data, databases, etc. are dump'd/tar'd/gzip'd to a remote location once a day, generally held as long as space permits.
