I'm pretty sure I managed to create one of these by accident when first using Linux, though with much messier results. Same root cause though: the ext2 and FAT superblocks didn't overlap, and back in the day some of the filesystem creation tools weren't so good at making sure to overwrite conflicting superblocks.
