Not to get all armchair psychologist, but this reads like like a potential case of autism (and there's nothing wrong with that, I'm just surprised I didn't read anything about it in there somewhere)
I'm so glad you're writing about your last experiences here, Jake. We seldom get a chance to really be present with someone so thoughtful, so articulate, so connected to their human experience, at the end of their lives; much less have it recorded for posterity. I'm sorry you are suffering so much, and to me it's truly amazing that you have the will (or discipline or whatever it is) to continue to pump out this "work" in the middle of it all. It's having a profound effect on me and I wanted to make sure you had a chance to know that while you're still alive to hear it. Thank you.
