The real issue is that the Navy never wants to blame senior leadership and prefers to blame the easiest targets; they also never want to blame vendors who they might want to buy something from again. The vendors of course never want to take responsibility for problems or they won't ever get another project. Congress never wants to deal with things because it might anger some powerful/wealthy constituent like a defense contractor who holds a lot of jobs. In the end there is no one that will take responsibility to say "this is crap, fix it" because everyone is more concerned with ruining their promotion/contract/influence. So you wind up with ordinary sailors and ship's officers bearing the brunt of this as they are expendable.
